I do not accept insurance. However, I will happily provide you with a detailed receipt of the services provided. You submit this receipt to your insurance company for reimbursement.
Time is a treasure and yours is just as important as mine. I will provide you with any information regarding what forms need to be completed prior to your visit so that we can make the most of our time together.
To be respectful to my other clients and to me, please notify me at least 24 hours in advance if you cannot keep your appointment. That time was set aside just for you and cannot be provided to someone else if you don’t give sufficient notice.
If you no-show or fail to provide less than 24-hours notice, you will still be charged full price. Of course, valid emergencies will be forgiven.
